What is mental health case management?

Mental health case management is a free service that helps you navigate the complex system of mental health/substance use care. Case managers can connect you with available resources and services for help when you experience barriers to access or have difficulty managing your condition.

A mental health case manager can help you:

  • Understand your individual health care plan and maximize your benefits
  • Coordinate care with providers, clinics, and programs to treat for your mental health or substance use condition
  • Connect to community-based services and resources for recovery, including shelter, food, transportation, and legal concerns
  • Enhance wellness through education, goal setting, and self-care
  • Develop a peer support recovery network
  • Define a safety plan for crisis management

Note: This is a non-clinical service. Our mental health case managers will support and have extensive background in mental health/substance use, but they're not licensed therapists. 

A behavioral health case manager does not provide:

  • Therapy
  • Crisis services
  • Help for finding a therapist, psychiatrist, or other mental health/substance use provider

Contact the Customer Care Center and request mental health case management services or use the Enroll now button and complete the online referral form. 
When completing the online referral form, select "behavioral health." Enrollment is voluntary and members can opt out at any time.
