Contact the Customer Care Center at 877-230-7555. We will be happy to mail the reimbursement form to you. It should arrive in three to five business days. Please complete the form, attach your receipt and return it to Prevea360 Health Plan.

Prevea360 Health Plan will process the claim within 45 days after receiving the receipt and claim form.

When you see a Prevea Health or Prevea360 Health Plan provider and receive your flu shot through a provider's office or clinic, the claim will be submitted by your physician and covered at 100% through your policy with Prevea360 Health Plan.

Yes. You can receive your flu vaccine anywhere, including non-plan providers, and receive the reimbursement. If you receive the vaccine from a non-plan provider, simply pay for the service at the time you receive it and send a copy of your receipt with the reimbursement form.
