We offer support before and after your procedure, providing education and coordination of services to ensure you receive the care you need. The case manager collaborates with the transplanting facility – especially with your nurse coordinator.
Case management begins with an assessment of your health and wellness needs to develop an individual care plan with personalized goals.
Your transplant case manager can help you:
- Understand and manage the condition that is leading you toward transplantation
- Coordinate care with providers, clinics, and programs to assist you through the transplant process
- Navigate your insurance benefits and understand how they pertain to you before, during, and after your transplant
- Understand whether appropriate prior authorizations for transplant services are in place
- Connect with our advance care planning social workers, if desired
